Birth Certificate

A birth certificate is a confidential document; therefore, the only people eligible to obtain the certificate are:

  • The individual
  • The biological or adoptive parents of the individual
  • The biological or adoptive grandparents of the individual
  • The guardian or temporary guardian of the individual
  • A legal spouse, next of kin, or personal representative of the individual's estate
  • An adult child of the individual

If you were born anywhere in the State of Georgia, you may request a certified copy of your birth certificate from the Liberty County Probate Court.

Death Certificate

A death certificate is a confidential document; therefore, only the immediate family of the deceased can obtain a death certificate, including:

  • The spouse
  • The biological or adoptive parents
  • The legal guardian
  • The siblings
  • The adult children

If you have a tangible interest in a matter pertaining to the decedent, you can request a certified copy of the death certificate; however, the cause of death and social security number will be redacted.

How to Request Certificate

By Appointment

You must contact the Liberty County Probate Court at 912-876-3635 between the hours of 8 am and 5 pm to schedule an appointment. Prior to the appointment, you must complete and submit the Certificate Request Form (PDF) along with a clear photo copy of a valid picture ID.

By Mail

You may also request a Georgia birth certificate or death certificate by mailing a written request to:

Requests by mail must include a completed Certificate Request Form (PDF), along with a money order made payable to Liberty County Probate Court and a clear photo copy of your picture ID.

Cost

The cost of obtaining a Georgia birth certificate or death certificate is $25 per person for the first certified copy and $5 per person for each additional certified copy. Only cash or money order is accepted.
